Overview

This document outlines the requirements for a system designed to manage incidents and claims related to liability, physical damage, and occupational accidental claims. The system will facilitate the recording, tracking, and management of incidents and claims, providing detailed financial and legal information as needed.

Features

Incidents

  • View and Convert Incidents: Ability to view existing incidents and convert them into claim types. Users can search for incidents by Incident Number, Description, Driver Name, Date, Location, Truck Number, Trailer Number, or Type of Incident.
  • Categorize Claim Types: Users can categorize claims by selecting applicable checkboxes for Incident Type, Driver, Date, and Location. Multiple incident types can be connected to one incident.
  • Incident Types:
    • Liability
    • Physical Damage with Other Party
    • Physical Damage for Truck
    • Physical Damage for Trailer
    • Occupational Accidental Claims

Claims

  • Filter Claims: Users can filter claims by Driver Name, Date, Location, Type of Claim, and Status. Multiple status choices can be selected simultaneously.
  • Claim Categories:
    • Liability
    • Physical Damage with Other Party
    • Physical Damage for Truck
    • Physical Damage for Trailer
    • Occupational Accidental Claims
  • Claim Details: For each claim, users can filter by Incident Number, Description, Driver’s Name, Date, Location, Truck Number, or Trailer Number. The system will notify users if multiple claim types are open for an incident, allowing access to all or selected claim types.
  • Edit, Print, and Download: Users can edit, print, and download all details under each claim section.

Analytics

  • Dashboard: Provides analytics for Occupational Accidental Claims, including written statements, photos/videos, medical confirmation, and witness information.

Financial Details

  • Basic and Additional Details: Financial details are split into Basic (Total Claim Paid Amount, Paid claim amount by Company/Insurance, Driver at Fault, Estimate Amount) and Additional (Cleaning/Debris Fees, Towing Invoice Amount, Rental Invoice Amount, Downtime Amount, Hotel and Travel fees).

Document Uploads

  • Upload and Rename: Ability to upload various documents related to incidents and claims, with the option to rename each document for clarity.

Legal and Financial Management

  • Legal Involvement: Capture attorney details, upload legal case documents, and manage legal lawsuit sections.
  • Financial Management: Detailed tracking of claim payments, deductible charges, and additional supplements.

Reportable Accidents

  • Accident Register: Capture details such as Driver’s First and Last Name, Accident Date, Time, Street Address, City, State, FMCSA Crash Report Number, Number of Deaths, Non-Fatal Injuries, Towaways, and Hazardous Materials involvement.
  • Additional Information: Upload Citation, Post-Accident Testing results, and Police Report if applicable.

User Interface

  • Claims Section: View and manage incidents and claims, filter by various criteria (Incident Number, Description, Driver Name, Date, Location, Truck/Trailer Number, Claim Type, Status).
  • Analytics Dashboard: Display analytics related to claims and incidents.

Constraints

  • Document Management: Ensure all uploaded documents are securely stored and easily accessible for review and editing.
  • Data Integrity: Maintain accurate and consistent data across all incidents and claims.
